2024 Campaign Supplies Order Form

Please complete this form to request campaign supplies. ArtsWave will ship supplies to the name and address you provide below. Please allow at least one week from the time of submission to receive your supplies.

We also have digital versions of our campaign materials, including the non-payroll pledge form, campaign brochure, posters, and Affinity and Networking Groups flyer. You can also request the creation of a online portal for your workplace campaign, or explore using e-pledge.

Please indicate the quantity needed for each of the following materials.

Please read the explanations under each item to guide your ordering. If you have questions, do not hesitate to reach out.

The brochure is meant to give an overview of who we are, what we do and the impact we have on the community. This should be requested for each employee solicited.
These are 18"x24" and can be placed anywhere employees will see them.
These are for members of your campaign team only. Please indicate numbers needed by size. (Example: M - 3, L - 2, etc.)
These are for companies who wish to keep the pledge information of their employees private.
Reminder: Please allow at least one week from the time of submission to receive your supplies. We will begin shipping in mid-January unless there is a special circumstance. Thank you for your submission!
